You MUST register on-line before turning in the permission slip. Registration for an event is a two step process (register on-line and turn in the permission slip and any fee to the Faith Formation office).

Print the permission form and turn in to the Faith Formation Office with fee (if applicable) by the deadline date. Only one slip is needed per family per event. Be sure to include the names of all family members who will attend on the permission slip. You may email a scan to:
