Registration and Sign Up for Mentors

Thank you so much for showing interest in sharing God’s love with teens at St. Jude through mentoring!  

Step 1: Please read through this Mentoring document and Agreement.

Step 2: Please fill out the on-line sign up form below.

Step 3: If you are new to volunteering with Youth Ministry, please attend a 15 minute interview (option of face to face and virtual) on September 8, 9, or 10.  (After your sign-up form has been received, an email will be sent to you with the times and logistics for the interviews).  

Step 4: Please submit your signed Mentor Agreement (drop it off at the office or email to

Step 5: Watch the Mentor Training Video

Step 6: Complete Protecting God’s Children Workshop and/or submit/update paperwork (if that is not already completed)
